HOSPITALIZATION

Hospitalization Procedure

  • Please come to the Hospitalization counter at the 2nd floor. We will give you an orientation for your hospitalization.
  • Please bring your Hospitalization permit, Medical examination ticket (ID card), Insurance card, Certificate of Eligibility for Ceiling-Amount Application and complete your hospitalization procedure by a specified time.
    • Please fill in the prescribed items on the hospitalization application form and submit it together with your hospitalization permit to the hospitalization counter.
    • If you are over 75 years old, please bring your Health Insurance card for the elderly.
    • We will return your insurance card immediately after confirmation.
    • Please show us your insurance card at the hospital counter at the beginning of the month, since it is necessary to check it monthly even during hospitalization.
    • Please note that if you do not present the necessary documents for health care application such as your insurance card, you may be required to pay the full hospitalization fee.
  • Patients who are foreign nationality and do not have valid Japanese health insurance,20 yen (including tax) will be calculated for each medical treatment fee. Therefore, if you do not have an insurance card, please show us your passport or residence card.
  • If you are subjected to Medical assistance, Rehabilitation medical care, Services and Support for Persons with Disabilities Act, Medical Treatment for Premature Babies, Specific medical care, Medical care for pediatric chronic specific illness, please let us know at the hospitalization counter.
  • After completing the hospitalization procedure, please come to the nurse station in the ward where you will be hospitalized.
NOTICE
  • If you are unable to complete the procedure due to Emergency hospitalization (outside of hours), please complete the hospitalization procedure at Emergency reception desk or at your earliest convenience the next day. (Next morning)
  • To change or cancel the hospitalization date, please contact your doctor in charge, the ward nurse or the outpatient department as soon as possible.

What to bring for your Hospital stay

  • Chopsticks, cups, forks and spoons
  • Toiletries (toothbrush set, soap, shampoo, hair conditioner, etc.)
  • Daily necessities such as tissue paper, Non-slip slippers, towels and bath towels (You can purchase some items at the convenience store)
  • Underwear (including changing clothes)
  • Sleepwear (If you use hospital gowns, we will charge you 74 yen per day.)

※Since the storage space is limited in the hospital, please keep your possessions to a minimum.

Things to do once hospitalized

  • Change to hospital gowns (74 yen per day) or your own sleepwear.
  • Please wear a certified wristband to prevent misidentification of patients. You need to confirm your name on the wristband with a nurse.
  • Please confirm the name of the doctor in charge with a nurse.
  • Other details of hospitalization will be explained by a nurse.
  • ※We will give you a pamphlet of “Inpatient Information” for the details.

NOTICE
  • Please refrain from parking your car at parking lot during the hospitalization because we need to keep spaces for outpatients.
  • It is permissible to bring communication devices into hospital rooms. We allow mobile devices such as smartphones which is necessary for daily life, but you are not allowed to bring in devices that are unnecessary for medical treatment.
  • You can use the TV or cooler box with a TV card. When using TV in your room, please ask your nurse or doctor. (Please use your earphones or headphones.) You can purchase a TV card at the vending machine.
  • Please do not bring in large amounts of cash or valuables (jewels, accessories etc.) with you to prevent theft or losing your belongings.
